To prepare for your first appointment, we’ve simplified the process by allowing you to complete new patient paperwork online before your visit. This saves you time and allows us to focus on your care during your appointment. Gathering any prior dental records and noting questions will further enhance your personalized experience.

Please download, print out, and fill out your new patient forms and bring them to your first visit for a hassle-free experience. Don’t have a printer? No worries! Please arrive 15 minutes early to your scheduled appointment to complete them in person.

Step 2: Prepare for your Appointment

To ensure your first appointment goes smoothly, we encourage you to prepare beforehand. Double check your bag to make sound decisions about your dental care and oral health.

  • Medical or Dental Insurance Card (If applicable)
