When wisdom teeth need to come out
Wisdom teeth are the last molars to come in, usually in the late teens or early twenties. When there is not enough room, they get stuck, or impacted, in the jaw or under the gum. Impacted wisdom teeth can lead to pain, swelling, stiffness in the jaw, and infection where bacteria get in around a partly erupted tooth (ADA MouthHealthy). Depending on where they sit, they can also cause headaches or earaches.
We see this most in young patients. About 9 in 10 of our wisdom-teeth patients are between 17 and 25. Now and then an adult comes in who never knew their wisdom teeth were still there, often a full-mouth restoration patient, and we remove them as part of that plan. How we plan it: pano and CBCT
It starts with imaging, either at a separate consult or in the same visit as your removal. We take a panoramic x-ray to see where the teeth sit, and a CBCT 3D scan to see exactly where the lower teeth are in relation to the nerve that runs through the lower jaw. The lower wisdom teeth sit far back, near the jaw joint, so knowing where that nerve is matters. CBCT is the most accurate way to judge how close an impacted lower wisdom tooth is to that nerve (Journal of Medicine and Life, 2015). The scan tells us whether the tooth can come out cleanly or needs a careful, staged approach.
Sedation that fits you
You do not have to be put to sleep to have wisdom teeth out. We offer:
- Local anesthetic: numbs the area. We have had patients do all four this way and do fine. Often about 30 minutes.
- Nitrous oxide: light relaxation, you stay awake.
- IV sedation: for patients who would rather not hear or feel the procedure. With sedation and the extraction together, simple cases are often done within 30 to 35 minutes.

Every case is different, so we give you a firm price once we see your scan, before any treatment. As a guide:

A lot of sedation is billed under medical rather than dental coverage, so patients with medical insurance often get more of it covered. One of our patients paid about $500 total for all four impacted wisdom teeth with sedation because her insurance covered most of it. Recovery and aftercare
For impacted teeth, Dr. Mancin often places sutures. The lower jaw leaves a larger open wound, and stitches help protect the site while it heals. The upper sockets tend to heal fast, so up top it is mostly taking it easy for a couple of days.
A dry socket happens when the blood clot over the socket is lost and the bone underneath is exposed. It is painful, and it is more common after surgical wisdom-tooth removal than after a routine extraction (JCDR, 2015). Smoking raises the risk, so do not smoke while you heal (same review). We also ask you to skip straws and chewing gum for the first few days, rest the first day, and keep the area clean.
What we send you home with:
- Soft foods for a few days: soups, mashed potatoes, anything easy.
- Warm salt-water rinses a couple of times a day, gently, not aggressively.
- Pain medication the day of your procedure.
- Rest. Especially day one. No hard activity.

Your one-week follow-up
About a week later you come back. We take follow-up imaging, a panoramic x-ray or another CBCT depending on what we need to see, to confirm everything is healing the way it should, check that the sutures are intact, and make sure the sockets, upper and lower, are clean and not infected. Food can pack into the sockets, so we clean those out if needed. This follow-up is included in your treatment fee.
Honest expectations
Wisdom teeth are wisdom teeth. They are not always easy, and every case is its own case. Everybody heals differently and everybody’s pain tolerance is different. With a full or partial bony impaction we are surgically removing a tooth, so the soreness afterward can be a little more than a simple extraction. We will tell you straight which kind of case yours is once we see your scan.
