To prepare for your first appointment, we’ve simplified the process by allowing you to complete new patient paperwork online before your visit. This saves you time and allows us to focus on your care during your appointment. Gathering any prior dental records and noting questions will further enhance your personalized experience.

Step 1: Complete New Patient Forms

Please download, print out, and fill out your new patient forms and bring them to your first visit for a hassle-free experience. Don’t have a printer? No worries! Please arrive 15 minutes early to your scheduled appointment to complete them in person.

Step 2: Prepare for your Appointment

To ensure your first appointment goes smoothly, we encourage you to prepare beforehand. Double check your bag to make sound decisions about your dental care and oral health.

  • Medical or Dental Insurance Card (If applicable)
  • A form of payment
  • Questions to ask our Dr. Mancin so that you don’t forget anything on your first appointment. You can also make a note of all the concerns you have so you can make sound decisions about your dental care and oral health.
